Those patients on the severe mental health register (e.g. those with a history of psychosis within 5 years) who are not under a psychiatrist will be offered an appointment to formulate a care plan.

Dementia
We offer an annual review with a GP or nurse for patients with a diagnosis of dementia and their carer/close family member where appropriate. This will include formulation of a care plan.
Diabetes
A Diabetes annual review is offered as standard by the practice after blood tests have been taken in your birthday month. Blood tests will be completed 6 months after. Based on your blood test results and diabetes management you may additionally be called for a 3 month or a 6 month review.
Chronic obstructive airways disease (COPD)
You will be invited for an annual review with our COPD nurses. Please ask for a telephone consultation with the nurse or ANP when you use your rescue medication (oral steroids +/- antibiotics) so they can arrange a replacement prescription for you to keep “to hand”.
Asthma
If you are well controlled you may be opportunistically reviewed by a GP or a nurse, if you are not under control you will have a review with our asthma nurse.
Pharmacy First Minor Ailment Scheme
Pharmacy First is a new minor ailments scheme that is running as a pilot in the area covered by Birmingham, Solihull and the Black Country Area team. It enables patients to access a free medicines service without having to visit their doctor, Accident and Emergency or the walk-in centre.
Patients who are registered at participating surgeries can access the service up to three times in six months for treatment for a variety of listed conditions:
